This report presents the results of our review of the Pipeline and Hazardous Materials Safety Administration (PHMSA) Office of Pipeline Safetys (OPS) oversight of hazardous liquid pipeline operators processes for mitigating and repairing threats to pipeline integrity. The objectives of our audit were to assess: (1) actions taken by pipeline operators to remediate integrity threats and (2) OPS efforts to verify the adequacy of these corrective actions. In March and April 2006, we presented some of our preliminary audit findings at two congressional hearings on pipeline safety. Exhibit A contains details on the scope and methodology we used in conducting this review. Low-stress transmission pipelines (the type that failed on Alaskas North Slope in March and August 2006) were not included in this audit because they are currently exempt from Federal pipeline safety regulations. PHMSA is working on safety regulations that would address these pipelines and issued a notice of proposed rulemaking on August 31, 2006. |
