A core mission of the Federal Aviation Administration is safety oversight - the process of ensuring that airmen, airlines, aircraft, manufacturers, and a host of others who are engaged in aviation perform their functions safety and responsibly. This report surveys the history of aviation safety oversight in the United States and how decision makers and stakeholders have perceived its performance over time. This report takes a generally chronological approach, dividing the history into seven main epochs. The effort was motivated by the premise that present efforts to re-engineer the oversight system, to be effectual, required understanding and respect for the how the present system evolved.
