摘要: About 4:38 p.m. central daylight time on October 12, 2003, westbound Northeast Illinois Regional Commuter Railroad (Metra) train 519 derailed its two locomotives and five passenger cars as it traversed a crossover from track 1 to track 2 near Control Point 48th Street in Chicago, Illinois. The train derailed at a recorded speed of about 68 mph. The maximum authorized speed through the crossover was 10 mph. There were about 375 passengers and a crew of 3 onboard. As a result of the accident, 47 passengers were transported to eight local hospitals. Of these, 44 were treated and released, and 3 were admitted for observation. Damages from the accident exceeded $5 million. The safety issues discussed in this report are the adequacy of the locomotive engineer.s performance, training, and qualifications, and the lack of a safety redundant system to address train crew performance deficiencies. As a result of its investigation, the National Transportation Safety Board issued safety recommendations to the Federal Railroad Administration and the Northeast Illinois Regional Commuter Railroad (Metra). The Safety Board also reiterated a previously issued safety recommendation to the Federal Railroad Administration. The National Transportation Safety Board is an independent Federal agency dedicated to promoting aviation, railroad, highway, marine, pipeline, and hazardous materials safety. Established in 1967, the agency is mandated by Congress through the Independent Safety Board Act of 1974 to investigate transportation accidents, determine the probable causes of the accidents, issue safety recommendations, study transportation safety issues, and evaluate the safety effectiveness of government agencies involved in transportation. The Safety Board makes public its actions and decisions through accident reports, safety studies, special investigation reports, safety recommendations, and statistical reviews.
