Flight safety depends on maintenance; however, poor maintenance is a major factor inmany aviation mishaps. This is due to the fact that some maintenance activities are carried outimproperly or are overlooked as a result of human error. Although maintenance staff areaccountable and responsible for their actions, it is important to acknowledge that maintenancemistakes are a visible sign of deeper organizational issues. Therefore, adequate solutions tomaintenance issues must consider organizational influences. Despite efforts reduce the accidentrate within Royal Saudi Air Force (RSAF), the RSAF suffers from an increasing trend inmishaps attributed to maintenance. Safety data was subsequently analyzed to gain anunderstanding of the problem nature and to discover trends within the data. Additionally, theHuman Factors Analysis and Classification System (HFACS) was used to investigate previousaccidents reports in an attempt to discover the root causes behind the accidents. HFACS wasapplied to 16 maintenance-related accidents to capture the nature of and connections amonglatent conditions and active failures, as well as uncover the underlying causes to the accidents.Twelve-hour shifts, fatigue, shift handover documentation & recordkeeping, and managementresponse to maintenance issues were among the underlying causes discovered in this research. |
